• Coordination of Supports

Encompass: Plan Connect provides Support Coordination for NDIS participants residing in Victoria. Our team currently services the following Local Government Areas: Greater Geelong, Surf Coast, Colac Otway, Wyndham, Golden Plains and Moorabool.

Our Support Coordinators will work closely with you to implement your NDIS Plan and work towards your goals. We will help you:

• To understand what’s in your NDIS Plan and how much you have to spend on the supports you need. We’ll work together to get the most out of your plan, even if it means getting a little creative.

– To connect you to all the providers that will give you the support you need. Don’t worry about the paperwork, we’ll sort that out for you.

– Talk to the NDIA for any reason. Sometimes you may want to request an earlier plan review, submit a complaint, or simply contact your Planner. We can help you by providing reports or just being by your side as you talk to the NDIA.

– Prepare for your plan reviews. And if you’d like, we’ll attend your plan review with you so you can be sure that the NDIA will have everything they need to know.
