CareWise Services is a registered NDIS Service Provider based in Melbourne’s West. We support and empower individuals to achieve their goals so they can live and enjoy full lives.

We can assist NDIS participants aged 7-65 with acquired brain injury, autism, physical/intellectual disability.

Our services are:

• Daily living activities
• Participation in community
• Household tasks
• Development of daily living & life Skills

Our approach is to always put the participant first and help them meet their needs and goals by working together with their families, carers and other support providers.

Our team is experienced, multi-lingual, can help with a range of activities/tasks and offers a person-centered care.
