Crescent Community Care is an approved disability service provider registered under the NDIS Scheme, which delivers care services to the clients with disability. We deliver a range of services and programs that may have been chosen and detailed in client’s NDIS Plan.

We also link client with mainstream community activities as well as disability-specific activities and services. Our disability programs offer a wide range of support services and group activities to help client engage with family, friends and the community no matter what type of disability they have.

Our support coordinators can assist you in building a circle of support around your specific needs and help navigate your NDIS Plan, focusing on getting the best value from your plan with creativity and flexibility. They can connect you with providers and supports that help you achieve your goals, such as housing, employment, education, health, and justice. Check in with you on your chosen providers to ensure they work well for you.
