Transport and Travel Support

We at Kelch Care understands that is important to have freedom to do the things you enjoy in the community and exercising that independence to get out and about in your community. We can support you to travel on the bus, train, in your car or provided car to visit friends and family, travel to your job, do some shopping and access health and education services.

Community Nursing Support

Our experience nursing staff provide NDIS participants from different cultural backgrounds the best of healthcare. Our nurses will work closely with your local health practitioners to battle illnesses and ensure health. Our nurses will identify barriers to healthy lifestyle and general wellness as they work with families and communities to empower individuals.

Accomodation & Tenancy Assistance

We provide expert assistance to participants to meet their housing goals, communicate with your real estate agents or proprietors to identify problems that risk tenancy, implement effective plan to maintain your current tenancies, look at alternative options of housing. We extend support with accommodation and tenancy by providing budgeting through liaising with utility providers and money care.

Psychosocial Recovery Support

Our experienced Psychosocial recovery coaches provide recovery support to NDIS participants with psychosocial disability using a customised recovery plan that will help you regain your confidence in the community, and assist you gain skills to live a full and contributing life in the community.

Our recovery coaches have many years of experience in providing mental health care in both community and hospital setting. They will support you through your recovery journey, educate you on how to manage your mental health challenges and liaise with other mental health agencies in the community. 

Assist Daily Task and Shared Living

We assist with and/or supervise daily task life with the goal of developing the skills of NDIS participant to live independently. The support is provided to each person living in the arrangement in accordance with their need. This may include participant’s living in the same household or in cluster units or participant’s living in a host family arrangement. This support occurs in respite accomodation and or long stay shared accomodation.

Individual House Hold Task

Without a plan for keeping the house clean, it can seem like the to-do list is endless: the spice rack is impossible to navigate, the towels constantly need to be washed, there’s a stain in the cupboard by the stove. Kelch Care will support you in overcoming those worries. We can support you to complete those household chores and help you break each task and work them in daily, weekly or monthly basis using a good daily support plan. 

Social and Civic Activities

Our social and civic activities are designed to help develop participant’s ability to socialise and participate in the community. Our activities like; health and wellbeing education, shopping skills, attending recreational activities  e.g tenpin bowling, picnics, arts and craft events, music festivals, attending cinemas and more. We can support you with; finding a social group or personal development course, transport to and from outing or holiday and support at the destination.

Support Coordination

We will assist you to build the skills you need to understand, implement and use your plan. Our support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

We can help to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.
