Perx Health is a digital care company supporting individuals to better self-manage and engage with their health. Our motivational programs help people living with chronic conditions stay motivated and complete their daily health tasks. Our Perx program for NDIS participants simplifies treatment and motivates self-management to achieve health goals and support independent living. Easy to download and set-up, we provide you with a single, flexible solution to self-manage your entire treatment plan including medication schedules, physical activity and exercise plans, appointment scheduling, measurement taking and health tasks. We help you to achieve your health goals through:
- Habit coaching and digital support to make health habits stick
- Community challenges and engagement with our Perx Community Manager
- The opportunity to win monthly rewards for completing your daily tasks
You will also have access to an online care portal allowing you to track your health goals and share progress with a carer, family member or healthcare professional (physiotherapist, occupational therapist etc.).
