No matter what your needs or aspirations might be, our NDIS Support Coordinators can work with you to access the support you need. We provide different levels of support coordination depending on your or your loved one’s needs
At Point Care, we understand that no two clients are the same. This means it’s essential to offer a customised service that’s tailored to the needs of the individual. Part of the customisation process includes offering clients a choice of three different support levels – support connection, support coordination and specialist support coordination.
The level of support you select is entirely up to you – we are always happy to explain what you’ll get at each level, as well as provide recommendations on the type of support that’s likely to give you the outcomes you’re looking for.
Support Connection
The main aim of the Support Connection level is to support our clients in using their NDIS plan more effectively. This may include putting them in touch with networks of service providers, or with large-scale support options that may be available locally. For many clients, this level provides vital information on the various choices for service provision in the area, offering the data needed for high-grade, informed decision-making. Closely tied to the needs of each client, we offer bespoke, results-driven advice and guidance.
Support Coordination
Support Coordination is a higher level of support that has the aim of creating a tailored package of support that is optimal for the client. Services include:
- Helping clients to understand their NDIS plan.
- Empowering and building confidence.
- Putting clients in touch with high-grade services that can meet their needs.
In a coordination role, we work with clients to understand their needs and how they want them to be met, then assist them in putting together a holistic package of support that covers all the aspects of care they need.
Specialist Support Coordination
If you or your loved one has complex or challenging needs, the Specialist Support Coordination package could be exactly what’s required. This specialist role includes responsive, pro-active support with managing health crises, in addition to targeted assistance in maintaining a wide-ranging programme of support.
We recommend Specialist Support Coordination for clients who require a high level of care and/or may require rapid intervention (such as clients who suffer from seizures or diabetes).
