Support Coordination

Let us assist you with implementing your NDIS plan and engage with services that will assist you with achieving your goals. We have vast knowledge of the NDIS and will also assist you with liasing with them to ensure that your plan reflects your current needs.

Recovery Coaching

At Sunflower we understand that the pathway to recovery is a journey and that we all experience some bumps along the way. Our services can support you to engage with services at your own pace by spending time getting to know you, link you in with services and help you better understand the NDIS and how it can support you in your recovery.

Support Worker Assistance

At Sunflower, we ensure that we are here to support you to live an ordinary life both in your own home and when out in the community. We are committed in ensuring that you and those around you are able to build a strong working relationship with workers who support you and that they will assist you to have as much independence as possible in your day to day life.

Counselling

We all need someone to talk to from time to time, and whether you have a mental health diagnosis or disability, or are a carer, it never hurts to have a chat. We offer a space for you to sound board your thoughts without judgement, help you establish your own goals and then assist you to work towards them at your own pace.

Occupational Therapy

Whether ongoing support is needed or you require a full functional assessment with recommendations for review we are able to assist. We are also able to provide reports for SIL and SDA supports as well as assistance with accessing required low cost as well as higher levels of assistive technology.

STA

Beautiful STA property available in Craigieburn Victoria. Come and be supported by one of our lovely staff, walking distance away from Leisure Center, cinemas, and shopping centre. Activities and meals also included and tailored to your needs and likes.

Send your loved ones to be supported by us whilst you access some much needed respite, knowing that they will enjoy themselves being supported by caring and experienced staff, accessing activities that they enjoy such as bowling, the zoo etc.
