The Only Support Coordination is a registered NDIS provider. Specialised in the disability sector across government, community and allied health services. Working to strengthen participants resolve, alongside participants to provide them with Support Coordination, Assist with daily living and Recovery coaching.

Support Provided: Specialised  Support Coordination including Recovery Coaching​​​

• Support me to start my NDIS plan quickly.

• Support me to understand my Plan.​​

• Specialising in allied health discharge planning to integrate me from a clinical setting such as hospital to home. ​

• Assist me back into my local community using strength-based individualistic approaches, tailored to my goals.​

• Support me to source and connect with support providers, including informal, community & mainstream.

• Obtain quotes, negotiate services to be provided.

• Develop service agreements and create service bookings with preferred providers, working collaboratively with me.​

• Support me within the scope of the role of a support coordinator with unexpected issues and build my resolve towards independence with choice and control at the forefront in everything I do. 

• Attend appointments with me to build confidence.

• Work with me and my support system by using my preferred mode of communication, as a team to build my independence and goals. 

• Prepare me for my plan review.​
