TK Community Care is a disability services organisation based in Darwin in the Northern Territory. Our Team is highly trained and dedicated to helping you create greater freedom and control over your support needs.

 

Supported Independent Living (SIL)

TK Community Care provides dedicated group accommodation homes focussed on supporting residents to live as independently as possible, with support provided by a team of experienced allied health professionals and support workers. The cost of the accommodation itself (e.g. rent, utilities, groceries, etc.) must be met by clients, however the professional support we provide in our SIL environments can be funded by the NDIS for eligible participants.

 

Respite

In addition to long-term accommodation, we can provide short-term and emergency respite support for families and carers, by providing high quality support for clients within our SIL accommodation.

 

In-home Support

Our in-home supports range from occasional domestic assistance such as cleaning and home maintenance, through to round-the-clock personal care, all delivered by committed and enthusiastic staff.

 

Support Coordination and Psychosocial Recovery Coaching

TK Community Care has a team of dedicated allied health professionals and support workers who will work in partnership with you to identify and link with the supports you need to live a meaningful life.

 

Assistance with Development of Daily Living and Life Skills

All of our services have an emphasis on helping you to live as independently as possible in the way you desire. We also provide a range of mentoring, training and development activities to help increase your capacity and ability to undertake the day-to-day activities you find important.

 

Participation in Community, Social and Civic Activities

We understand being involved is important and realise the value in being part of a group and sharing enjoyable time together. Our support workers will provide you all the assistance you need, alone or with a group, to be actively involved in your community. We will also work with you to identify innovative ways to help you experience all your local community has to offer.

 

Community Nursing Care

Our Registered Nurses are highly trained and will deliver the specialist care you need when you need it, in collaboration with your GP or other health professionals.

 

Counselling and Other Therapeutic Supports

We can provide specific support to help you develop your functional skills, such as in the areas of language and communication, personal care, mobility and movement, interpersonal interactions and community living.

 

Assistance with Travel and Transport

Whether you need help getting to school, work, an important event or just that next appointment, we will help you (and your carer) get from A to B.

 

Specialist Positive Behaviour Support

We provide positive behaviour support including assessment and the development of a comprehensive plan to help you.
