I am writing to express my interest in working with you and your current participants requiring a
NDIS Provider, the services we provide are:
• Supported Independent Living (SIL)
• Social and Community Participation
• Therapeutic Supports
• Travel and Transport
• Personal Care and Daily Tasks Assistance
Our current qualifications and experience include Master of Advanced Social work, Cert iv
in Disability, working with children check, 5+ years working in Disability Services and over
ten years of experience providing high quality care and support to vulnerable adults,
children, families, and communities.
We are located at Ballarat and able to take on any self or plan managed participants within Ballarat
and environs. Yale Care Services Pty Ltd. is home based although we are happy to travel to the
participants to provide services.
Our official Audit has been approved and we are waiting for registration number from NDIS
Commission.
Our operating hours are 24/7 and has started taking on new participants as of 19 June 2023.
